   1. That the Requesting Party is a duly authorized representative of the entity they represent.
   2. That based on the personal knowledge of the Requesting Party and their due diligence, they or the entity they are representing has not initiated a request for advice or opinion from any government entity, quasi-judicial agency, or any other body (including voluntary dispute settlement entities) pertaining to the parties and issues referenced herein.
   3. If there is a pending advice or opinion from the foregoing entities, the Requesting Party shall provide the name of the government entity, quasi-judicial agency, and other body, and include details or information of the same in the form herein.
   4. That the Requesting Party consents to the publication of excerpts from the Non-Policy Matter Opinion on the PPP Center’s website for transparency purposes.
   5. That for purposes of securing a NPM Opinion, the Requesting Party also acknowledges and accepts the terms and conditions for requesting NPM Opinions from the PPP Center and understands that the Center may refuse to render an Opinion as provided in the applicable provisions of the Guidelines or Procedures for the request of Non-Policy Matter Opinions.

Privacy Notice:

All information we collect shall be kept private and confidential by the PPP Center and shall be used solely for legal purposes as mandated by the Data Privacy Act and other relevant laws. Information that are matters of public interest, however, may be disclosed to the public subject to applicable laws, rules, and regulations.

Only authorized PPPC personnel has access to these personal information, the exchange of which will be facilitated through email and hard copy. Storage shall be within a period as may be authorized by law. Physical records shall be disposed through shredding, while digital files shall be anonymized.
